with "Conditional" status (i.e., when the litter is not otherwise eligible
for registration with the
WSSBA).

(Pages 1 and 2) one time.
Complete the Conditional Litter Placement Portion (Page 3), which holds
placement information for three puppies, as many time as you must
to document the entire litter.

WSSBA Litter Health Report (LHR)
Have your Veterinarian complete this form when
puppies are examined/inoculated at 6-8 weeks of age.
You must submit a copy of this form
(or a comparable Litter Health Report) with your
WSSBA Litter Application & Placement Form or
WSSBA Conditional Litter Application & Placement Form.
WSSBA Puppy Aptitude Test (PAT)
The Puppy Aptitude Test (PAT) is used to evaluate puppy temperaments
at around 7 weeks
of age. Feel free to use this form as part of the process.
While the WSSBA does not require that puppies undergo a PAT,
we strongly recommend
that this be done by an experienced tester. |
